Complete Health Assessment

Complete a quick consultation to determine eligibility. Answer all listed questions. If approved, you'll pay the consultation fee and receive a Letter of Medical Necessity.

You can use your HSA/FSA card for the consultation payment.

Make Your Purchase
​
Return to the Corewell Health and Wellness Center website to make your purchase using a standard payment method.
Do not use your HSA/FSA card to pay for membership or services - debit/credit payment only.

Submit for Reimbursement
Follow Flex's instructions to submit your Letter of Medical Necessity and receipt to your HSA/FSA administrator.
Claims are typically paid out in 1-2 weeks.
